The patient receives visits including physicians, nurses, hospice aides, chaplains, counselors, social workers & volunteers- based on the plan of care. Visits may be every day, a few times a week, once a week, or whenever the patient needs. This service includes medications, equipment, and supplies related to the patient’s hospice needs.

This is continuous care that assists in relieving acute medical symptoms. The care keeps patients at home and out of hospital. The care happens during brief periods of crisis, as medically needed. One example would be if the patient needs constant checks by a nurse, hospice aides will come to home in shifts of up to 24 hours a day.
