Hospice Physician
Hospice Physician collaborates with the interdisciplinary team to finalize care plan, ensuring it aligns with the patient's goals & preference. He evaluates patient's medical condition, manages symptoms, prescribes medications, and recomends necessary tests & procedures.
Registered Nurse
The RN oversees the care plan, coordinates physical care, monitors patients for symptom management, and arranges medications, supplies, and medical equipment. Additionally, he/she educates patients and their families, what to expect, and how to provide appropriate care.
Certified Nursing Assistant
Certified Nursing Assistant (CNA) provide hands-on assistance under the supervision of Registered Nurse and other healthcare professionals. She aids with bathing, personal care, meals, mobility and exercise, laundry and running errands.
Bereavement Counselor
Bereavement Counselor supports individuals and families as they navigate the grieving process. The purpose is to offer compassionate counseling to help them cope with their loss, explore their feelings, and find healthy ways to express their emotions.
Social Worker
Social workers provide emotional support and counseling to individuals and families in hospice care as they transition to end-of-life care. They assess for distress and address psychological, social, and resource needs.
